The MDT (Multidisciplinary Team) program funded by the NTPHN commenced in July 2025. The aim of the program is to reduce hospitalisation and improve general wellbeing for people with diabetes, pulmonary disease and/or cardiac disease.
It is available for people living in the Darwin or Alice Springs region who:
- hold a valid Medicare card
- have diabetes, pulmonary disease, and/or cardiac disease
- be at high risk of escalation or progression of one or more of their chronic conditions in the next 24 months
- hold a current Health Care card, Low Income Health Care card, Pension card, or meet other specified financial hardship criteria*
- have support from their GP to participate in the program.
* Financial hardship criteria
- someone who has recently lost employment but does not yet have the required documentation
- someone who is experiencing family violence
- someone who is experiencing homelessness, housing insecurity or living in crisis accommodation.
The program is specifically for people who need extra help in managing their condition through access to free allied health services
Allied health services include:
A Care Co-ordinator helps with managing your referrals and appointments and provides ongoing support throughout the program. They work with you, your GP and the allied health providers to get the best outcomes.
Move & Improve
A 10-week low intensity physical activity program called Move & Improve is available at our Darwin office. Our exercise physiologist will individualise the physical activity to your needs and capabilities.
